CFAP Patient Notice of Change

Dear patient,

As a part of the journey of joining Mindpath Health we are excited to share changes in how you connect with your provider for your telehealth appointments. This change allows us to provide you with more mental health resources, empowering you to live your most fulfilling life. 

Patient Portal

CFAP Providers will still use the patient portal through AdvancedMD.  With our change to Mindpath Health’s Electronic Medical Records (EMR) you will receive an invitation to register based on your next upcoming appointment beginning the week of December 6th, 2022.  Just as before the patient portal allows you:

  • Access your records
  • Request a refill and access your prescription records
  • Message your provider and/or care team
  • Ask billing questions

Keep an eye out for an invitation via email. If you do not receive an invitation by December 12th, 2022 you can visit mindpath.com/patient-portal to create your account.

Online Visits

We will start using Mend platform for online/telehealth appointments. All virtual appointments scheduled on or after December 12, 2022, will take place in Mend.

This is different from the patient portal mentioned above.  

Look for the following important notifications via email and text:

  • Invitation to set up your Mend account
  • Request from Mindpath Health to complete insurance information and credit card details for payments
  • Notification for every scheduled appointment.
        • You will need to confirm each appointment confirmation request.
        • Appointment reminders will continue until the appointment is confirmed. Once confirmed your next reminder will be provided 30 minutes before your schedule appointment.

You can opt out of reminders by clicking the link in the email or replying to the text “STOP”.
